When my husband’s doctor was discharging him, he was so helpful for arranging continuing care. He set up a nurse to assess and change my husband’s wound dressing every couple of days. He also took the time to refer us to additional home services.
In some cases, a hospital discharge planner or a homecare
provider will recommend supplies you may be required to buy. An occupational
therapist (OT) may be available to either visit the person’s home or discuss how
things can be set up in the home.
Depending on the situation, the person may be able to receive additional home services such as home oxygen, community nursing, and physical therapy. The type of service and the amount of time given may depend on the program’s resources.
